Middle/High Students: 4:30-6:00

Wednesday October 7th at Griggs Road for students that attend Griggs Road, CVA, NGC, and CHS.
Forms for Flu Clinic

Please fill out the flu clinic form and bring with you to the clinic. Please do not send the forms into school with the student. If you need a printed form, please send Nurse Sciba an email and she will send one home with your student.

Flu Clinic Information
SC DHEC will return to our schools to give flu vaccines this year, but a few things will be different from previous years:
- You will need to remain in your vehicle with your student until you are called
- You will need to bring your own pen with you
- Everyone is REQUIRED to wear a face covering
- The clinic will be located outside in front of the school
- All the necessary safety precautions will be taken to ensure everyone's safety
- Flu Clinics are only for students currently enrolled in school
- Complete the form prior to arrival at the school and bring it with you on your scheduled day
