    All students entering school are REQUIRED by State law to have a South Carolina State Certificate of Immunization (DHEC 4024). 
     
    If any required immunizations are missing, parents/guardians will receive a DHEC 1123 with an expiration date that is 30 days from the date issued.
     
    If a SC Certificate of immunization is not turned in by the expiration date, your student will be suspended from school until they have received the required immunization(s).
     
    If you need your student's immunizations transferred onto a SC State Certificate of Immunization, please contact your medical provider or a York County Health Department.
