District Facility Usage Policy

  • Please answer each question and include all necessary insurance and financial documentation. Only correctly completed requests can be processed.

    Requests must be received no less than two (2) weeks prior to the first event date, to be processed.

    Do not contact school or site being requested, approval can only be given from CSD from facility rental and use.

    Once form is received, availability of site and personnel will be confirmed and cost will be calculated based on user status. An email will be sent with an explanation of decision.

    Upon approval, an invoice will be generated and is due upon receipt.
