Proof of Residency Information
New Home or Rental Agreement: Complete Section A of Verification form and…
- If you are in the process of building a new home you must provide to your zoned school:
- A signed contract with a closing date that is at least 8 weeks out from the date you register for school, and
- A copy of your Driver’s License/ID, and
- Once you are moved into your new home, you must provide the required two (2) proofs of residency to the school. One must be primary.
- If you have a Rental Agreement or Lease: You must provide to your zoned school:
- A copy of the signed rental or lease agreement and a copy of your picture ID
- Most recent utility bill (you will have 30 days from the date of registration to provide this to the school if you do not have it at the time of registration.)
**Should the move not take place as anticipated, the student may be required to return to his/her district of residence or pay tuition until such time as the move is completed**
New to the District: Living with a Homeowner or Renter (meaning: the parent/guardian of the student/s do not have a primary proof in their name): Complete Sections A & B of Verification Form and…
- If the parent/guardian registering the student does not own the home, you must provide to your zoned school:
- Two (2) recent (within the past 30 days) proofs of residence (one must be a Primary Proof) from the home owner (see list below), and
- Two (2) recent (within the past 60 days) proofs (both can be secondary proofs) of residence from the parent/guardian with that same address (see list below), and
- Copy of the parent/guardian Driver’s License/ID
- If the parent/guardian cannot provide two (2) proofs of residency at the time of registration, he/she will have 30 days from the date of registration to provide those two (2) proofs to the school.
- The Verification of Residence form must be notarized

- If the student is returning and nothing has changed as far as residency (same address, same parent/guardian…), you must provide to your zoned school:
- Two (2) recent (within the last 60 days) secondary proofs of residency (see list below)
- The Verification of Residence Form with notarization
