2022-2023 Registration Forms 

• 22-23 Emergency Card Information - Must be completed for each student attending OES.

Complete the following forms that apply to your student.

• 22-23 Bus Registration - Any student riding AM or PM must complete

• 22-23 Car Rider Pass - Any student that will be a PM Car Rider must request a car tag by calling the front office at (803) 631-8400.

  • You may only request car tags for students that you are parents or legal guardians of. Car passes will only be distributed to parents/legal guardians. Reminder: Students will not be allowed to enter a car without a visible each day. For safety reasons, those who do not have a car rider pass will be asked to provide proper identification.
