Immunizations
All students entering school are required by State Law to have a South Carolina State Certificate of Immunization (DPH 4024).
Special Exemptions:
If any required immunizations are missing, a Certificate of Special Exemption (DHEC 1123) will be issued allowing the student to continue to attend school until the required immunizations can be received or their records can be transferred from another location.
The special exemption expires 30 days from the date issued and can only be issued to the student once.
If a SC Certificate of immunization has not been turned in to the school by the expiration date on the Certificate of Special Exemption (DHEC 1123), your student will not be allowed to attend school until they have received the required immunization(s) or exemption(s).
Religious Exemptions:
A South Carolina Certificate of Religious Exemption (DPH 1126) can be issued in some circumstances. Please follow the link to the right to learn more.
Obtaining Immunization Certificates:
Students who receive immunizations from South Carolina providers and are up to date on all of their required vaccines can access their immunization record through the SIMON public portal.
If your student received their immunizations outside South Carolina a copy of their official immunization record may be entered with your consent into the SIMON immunization registry by the local health department, a SC medical provider, or their school nurse.
